2026 ORYX Performance Measurement Reporting Requirements

NEW for 2026:

  • Effective CY2026, The Joint Commission has added the following eCQM as an available optional measure towards meeting ORYX requirements:

    • eCQM: PC-08 Timely Treatment of Severe Hypertension

  • Effective CY2026, The Joint Commission has retired the following measures:

    • eCQM: Safe Use of Opioids - Concurrent Prescribing

    • eCQM: OP-ExRad Excessive Radiation Dose or Inadequate Image Quality for Diagnostic Computed Tomography (CT) in Adults (Outpatient)

    • eCQM: HH-PI Hospital Harm - Pressure Injury

    • eCQM: GMCS Global Malnutrition Composite Score

    • eCQM: HH-ORAE Hospital Harm—Opioid Related Adverse Events

    • eCQM: OP-40 ST-Segment Elevation Myocardial Infarction (STEMI)

    • eCQM: PC-01 Elective Delivery

    • eCQM: PC-05 Exclusive Human Milk Feeding

    • eCQM: STK-2 (eCQM) Discharged on Antithrombotic Therapy

    • eCQM: STK-3 Anticoagulation Therapy Atrial Fibrillation/Flutter

    • eCQM: STK-5 Antithrombotic Therapy by End of Hospital Day 2

    • eCQM: VTE-1 Venous Thromboembolism Prophylaxis

    • eCQM: VTE-2 ICU Venous Thromboembolism Prophylaxis

    • Chart: ED-1 Median ED Arrival to ED Departure-Admit

    • Chart: SUB-2 Alcohol Use Brief Intervention

    • Chart: SUB-3 Alcohol & Drug Use Treatment at Discharge

    • Chart: TOB-3 Tobacco Use Treatment at Discharge

  • Effective CY2026: Small Hospitals & Critical Access Hospitals are required to submit:

    • eCQM Hospital Harm: Hypoglycemia

    • eCQM Hospital Harm: Hyperglycemia

  • There are four additional available optional measures for small and critical access hospitals that have obstetrical services:

    • eCQM PC-02 Potentially preventable (NTSV) cesarean births

    • eCQM or CAM PC-06 Unexpected Complications in Term Newborns

    • eCQM PC-07 Severe Obstetric Complications

    • eCQM PC-08: Timely Treatment of Severe Hypertension

  • Effective CY2026: Large Acute Care Hospitals with Obstetrical Services have three specified required perinatal care eCQMs plus two additional specified required hospital harm eCQMs; Effective CY2026, Large Acute Care Hospitals with Obstetrical Units are required to submit :

    • eCQM Hospital Harm: Hypoglycemia

    • eCQM Hospital Harm: Hyperglycemia

    • eCQM PC-02 Potentially preventable (NTSV) cesarean births

    • eCQM or CAM PC-06 Unexpected Complications in Term Newborns

    • eCQM PC-07 Severe Obstetric Complications

  • Effective CY2026: Freestanding Psychiatric Hospitals only have two available and required measures; Effective CY2026, Freestanding Psychiatric Hospitals are required to submit:

    • Chart-abstracted HBIPS-2: Physical Restraint Use

    • Chart-abstracted HBIPS-3: Hours of Seclusion Use

There are no additional available optional measures.

  • There were no changes to the ORYX Performance Measurement Requirements for Assisted Living Communities (ALCs) for CY2026. ALCs are required to submit all four quarters of:

    • Chart-abstracted ALC-01 Off-Label Antipsychotic Drug Use

    • Chart-abstracted ALC-02 Resident Falls

    • Chart-abstracted ALC-03 Resident Preferences and Goals of Care

    • Chart-abstracted ALC-04 Advanced Care Plan/Surrogate Decision-Maker

    • Chart-abstracted ALC-05 Staff Stability

There are no additional available optional measures.

[CAH] Critical Access Hospitals adhere to the CAH standards manual, and are a specific designation by the state as a critical access hospital in a rural area.

[HAP PSYCH] Freestanding Psychiatric Hospital
HAP PSYCH are defined as a "Freestanding" Psychiatric Facility licensed and separately accredited as freestanding psychiatric hospital; it is not a unit within a hospital that is surveyed at the same time as the hospitals and shares an HCO ID #.
