ACCREDITATION

Dear Colleague,

The Joint Commission will be hosting office hours for the Direct Data Submission Platform (DDSP) on Tuesday, January 27, 2026 from 11am - 12:00pm (Central Time). The Direct Data Submission Platform (DDSP) is used for submission of required ORYX Performance Measurement data for accreditation. 

Office hours are a webinar-based format.   The content of the webinar for office hours includes tips for successful use of the platform and data submission.  Attendees can ask Joint Commission staff questions during the webinar via the Ask a Question feature.

This email is sent to all organizations with ORYX® performance measure reporting requirements regardless of the organization’s current CY2025 data submission status. 

We strongly encourage organizations to submit their data well in advance of deadlines to allow time to verify their data is entered accurately and on time. Organizations who do submit their data early have the benefit of Joint Commission staff assisting with troubleshooting issues if needed. 

Organizations electing to use consultants or vendors to assist with data submission on the DDSP should have their own staff log into the platform to verify their data has been submitted as expected. Joint Commission staff cannot confirm this for you.

Users with access to the DDSP should download and retain the Submission Report from the Home page on the DDSP, which provides a summary / proof of submission of the data received from your organization. Screenshots of data entry / uploads / submission pages are not accepted as proof of submission.

Upcoming Deadlines 

The deadline to submit CY2025 eCQM data is March 16, 2026 by 11:59 PM Central Time. Acute Care Hospitals and Critical Access Hospitals are required to submit eCQMs for the 2025 calendar year. There are no individual extensions and eCQM data cannot be resubmitted.

  • The eCQM Submit button is now available in the eCQM Module on the Submit Data page.

  • New Warning Logic on the Submit Data Page – Upon submission, a warning message will appear if the submission is not meeting the requirements for the program assigned in DDSP.  Please see the help topic eCQM Submit Data via the “Need Help?” link for more information.

  • Uploading eCQM data is not the same as submitting the data to Joint Commission – visit the eCQM Submit Data  in the eCQM help topics via the “Need Help?” link for more information.

  • PC-06 Attestation Reminder – if your organization attested to submit PC-06 as an eCQM in the chart-abstracted module, please remember to upload and submit PC-06 eCQM data.

  • PC-07 Present On Admission (POA) – for organizations submitting PC-07 data: Failure to include POA values for Severe Maternal Morbidity (SMM) diagnosis codes lead to inaccurate PC-07 rate calculations and negatively impact risk adjustment for PC-07; visit eCQM Measure Specific Resources via the “Need Help?” link for more information.

 

The deadline for entering 4Q2025 chart-abstracted measure data to the DDSP is March 31, 2026 by 11:59 PM Central Time. This is applicable to organizations required or electing to submit chart-abstracted data.

  • Reminder: Do not enter zeros for measures you are not collection/submitting data for

  • Organizations can use the resubmission function to submit incorrect or inadvertently omitted chart-abstracted data until March 31, 2026. Performance measures retired for CY2026 cannot be resubmitted past this deadline.

Refreshed Accelerate PI™ Dashboard Reports containing data through fourth quarter of 2024 are available for accredited hospitals and critical access hospitals. Data in the reports comes from chart-abstracted quality measures and electronic clinical quality measures (eCQMs) data reported by hospitals to Joint Commission under the ORYX® program.  

The dashboards are intended to be a springboard for conversations on data, performance measures, and quality improvement during the survey process. 

Accelerate PI Dashboard Reports are located under the Resources and Tools menu below the DASH heading in JC Connect®.

 

Removal of CY2024 QRDA-I files from the DDSP   

In accordance with Joint Commission’s data retention policy, the CY2024 QRDA-I files will be removed from the DDSP servers effective October 31, 2025. If your organization needs to download or review the CY2024 eCQM data, please do so before that date.   

The CY2024 Data Submission Report will remain accessible for viewing and download from the DDSP Home page. CY2025 eCQM data uploads will continue to be securely stored in the DDSP database and is not affected by this file removal.   

Action Required: 

If your organization needs to download or review your CY2024 eCQM data, please do so before October 31, 2025.

The Direct Data Submission Platform (DDSP) CY2025 eCQM module is now available.

From September 26, 2025 to December 31, 2025: Users are able to upload, review, purge and re-upload eCQM data.

From January 1, 2026 to March 16, 2026: Users are able to submit eCQM data.

The deadline to submit CY2025 eCQMs is March 16, 2026. We strongly encourage HCOs to submit well in advance of the deadline in order to have time to resolve any unexpected issues. Organizations required or electing to submit eCQMs to the Joint Commission must submit all four quarters of CY2025 data to meet 2025 ORYX requirements.

Dear Colleague,

Joint Commission updated the ORYX® Performance Measure reporting requirements for Joint Commission–accredited hospitals and critical access hospitals effective January 1, 2026.

In response to healthcare organization feedback, Joint Commission has updated the program to focus on critical patient safety areas while reducing the reporting burden by streamlining and tailoring measures based on hospital services. We reduced the number of measures available, focusing on measures that address three priority areas: maternal health, patient safety, and psychiatric hospital care. Additionally, we reduced optionality in the measure set, with fewer reporting requirements overall. The measures included for the 2026 program have strong scientific backing. These measures are either outcome measures or process measures where evidence clearly links the care processes to better patient outcomes. 

The updated ORYX® program requirements will allow hospitals and surveyors to learn from a more robust and representative data set. The standardized ORYX® measures closely align with what hospitals already report to CMS or other agencies, resulting in reduced measurement burden and a shared focus on topics that matter. Finally, we are enhancing our data infrastructure, including future updates to our data submission platform and analytics products.

Key CY 2026 updates to submission requirements

  1. Large Hospitals with ≥26 Licensed beds OR ≥50,000 Outpatient visits AND Provide Obstetrical Services

Required Measures:

·     HH-HYPO: Hospital Harm: Hypoglycemia

·     HH-HYPER: Hospital Harm: Hyperglycemia

·     PC-02: Potentially preventable (NTSV) cesarean births

·     PC-06: Unexpected complications in term newborns*

·     PC-07: Severe Obstetric Complications

Optional eCQMs:

Measures removed from ORYX:

With the goal of streamlining the measure set and reducing optionality, we will stop collecting several measures for the 2026 ORYX program. In prior years, hospitals reported a combination of required measures, and a set number of measures selected from a menu of available measures. In addition, NHSN Joint Commission group participation will not be required for 2026. We recognize NHSN is a valuable data source, and we will consider this data for future program requirements.

The Specifications Manual for Joint Commission National Quality Measures, version 2026A (applicable starting with 1/1/2026 encounters) and associated Release Notes have been posted to the Wiki Joint Commission Specifications Manual website  and the Joint Commission  website .
 
The 2026A Specifications Manual contains the following changes:

  • HBIPS - Updates to the HBIPS IPP and Algorithms were made to provide abstractor clarification and to align with the updated technical guidance provided in The Joint Commission Guide for Data Entry of Chart-Abstracted Measures.

  • ASR and CSTK - The rationale for the measures and references were updated to address 2025 stroke statistics from the American Heart Association.

  • PC-06 - Changes were made to the measure concerning transfers to another facility or other healthcare facility. Previously, all transfers to other facilities were severe complications. Transfers will now go through the algorithm to assess for severe complication codes or no codes, which will be severe complications, and moderate complication codes, which will now be moderate complications.

  • THKR - Updated Table 14.10 name to be more inclusive of additional fracture codes added. Added new data element Pre-Collection Mortality.

  • Appendix A Code Tables - Multiple tables were updated to include the 2026 ICD-10 updates.

The Joint Commission deadline for submitting 1Q2025 chart-abstracted data to the DDSP is June 30, 2025 at 11:59 PM Central Time. We strongly encourage organizations to submit their data well in advance of the deadline. This reminder is sent to all organizations regardless of their respective ORYX requirements or submission status.

IMPORTANT NOTE: if an organization is required but unable to submit eCQMs to The Joint Commission for CY2025, they must submit an extenuating circumstance request (ECR) form by June 1, 2025. If an ECR is granted, a minimum of three (3) chart-abstracted measures must be submitted for the entire calendar year, unless the request granted is for an exception for submission of any data.

To ensure the user list for your organization’s DDSP workspace remains current, please review all users with access to the DDSP for your organization(s), including any vendors/ consultants you may use and:

  • update user permissions as necessary

  • remove users that are no longer at the organization

  • invite additional users if needed

Security Administrators are responsible for managing user’s access to the DDSP and should review the user’s listed on the DDSP on a regular basis via the DDSP “Invite & Manage Users” page.

The 4Q2024 chart-abstracted deadline has not changed; The deadline for submitting 4Q2024 chart-abstracted data to the Joint Commission via the DDSP is March 31, 2025 at 11:59 PM Central Time.

This is a reminder for those organizations required or electing to submit 3Q2024 chart-abstracted data to The Joint Commission. Please note this email is sent to all organizations regardless of their current submission status within The Joint Commission Direct Data Submission Platform (DDSP).

The deadline for submitting 3Q2024 chart-abstracted data to the Joint Commission via the DDSP is December 31, 2024 at 11:59 PM Central Time. We strongly encourage organizations to submit their data well in advance of the deadline.

CY2024 ORYX Performance Measurement Timeline

  • 1Q 2024 (Jan, Feb, Mar) Chart-abstracted measures: DDSP opened February 1, 2024; deadline to enter data was June 30, 2024

  • 2Q 2024 (Apr, May, Jun) Chart-abstracted measures: DDSP opened May 1, 2024; deadline to enter data was September 30, 2024

  • 3Q 2024 (Jul, Aug, Sep) Chart-abstracted measures: DDSP opened August 1, 2024 and data must be entered by December 31, 2024

  • 4Q 2024 (Oct, Nov, Dec) Chart-abstracted measures: DDSP opened November 1, 2024 and data must be entered by March 31, 2025

  • CY2024 eCQMs: DDSP opened August 5, 2024; Transmission period to submit data opens January 1, 2025, and data must be submitted by March 17, 2025

Deadline for retransmission of 4Q2023 CAM Data
If your organization identified incorrectly entered or incorrectly omitted chart-abstracted data for 4Q2023, the deadline to correct this is December 31, 2024 at 11:59 PM Central Time. There will be no opportunity to resolve 4Q2023 CAM data beyond this date.

Dear Colleague: 

This communication is to correct an error on Slide #89 from the 9/10/2024 Office Hours. The slide stated that eCQM patient-level data submitted to the DDSP will be removed six months after the close of the eCQM submission period. However, sub-bullet #3 incorrectly stated that “QI page graphs and results table view… will continue to display.” The correct information is that the data associated with the graphs on the QI page and the results table view will not be available after the patient-level data has been removed from the DDSP. 

Historical HCO-level aggregate eCQM data is included in the DDSP’s Data Submission Report. This report is available on the Home page of the DDSP and documents the hospital’s data submission for the year. 

Hospitals wishing to retain the DDSP-generated eCQM patient-level data results must download them from the DDSP Quality Improvement page before six months after the close of the eCQM submission period. The patient-level results can be downloaded using the “Review Patient Results” button on the Quality Improvement page. For CY2023 data, the deadline to download is September 30, 2024.

The Specifications Manual for Joint Commission National Quality Measures, version 2025A (applicable starting with 1/1/2025 encounters) and associated Release Notes have been posted to the Wiki Joint Commission Specifications Manual website and the Joint Commission website.
 
The 2025A Specifications Manual contains the following changes:

  • The use of Gestational Age code tables in PC-01 and PC-02 has been removed from the 2025A manual since the intent of the measures is to capture the gestational age at time of delivery.

  • The PDF version of the manual is not being published due to 508 Compliance issues.  As an alternative option, users can PDF each section of the web version by clicking on the ellipsis in the browser menu, then click Print and select Adobe PDF.
