eCQM Data Accuracy

Purpose:Â
When selecting the 'Data Accuracy' from the menu page the ‘Data Accuracy’ page automatically opens and provides the ability to review:
The QRDA I documents that received warning messages - these messages may point towards potential incorrect eCQM evaluations. However, documents with only Warning messages may still be submitted by using the Submit Data page.
The QRDA I documents that received error messages and have been subsequently rejected. If possible, they must be corrected and re-uploaded as they cannot be submitted to The Joint Commission and will affect your hospital's measure rate(s).
The page provides the ability to access the Source Viewer for all documents that generated a warning and/or error message.
This page is intended to be used by users with knowledge of the structure and format of the QRDA I documents as well as the content within them. Familiarity with the QRDA I technical documentation will assist users of this page in identifying the reason why QRDA I documents were rejected or warnings generated.
Results will appear after data is loaded and processed:
The Upload Data page may show that zip files have been uploaded to the DDS Platform; however, this page may display the message "Results will appear after data is loaded and processed".
Confirm that the uploaded zip file did not generate any high-level platform errors by reviewing the ‘Data Details’ on the Upload Data page.
If no high-level errors were generated by the file, then your zip file is in the queue for processing against the Schematron, edits, and eCQM logic.
The processing turnaround time is dependent upon how many other zip files (including from other organizations) are waiting in the queue for processing.
If you continue to receive this message after 24 hours, please create a DDS Platform Support Ticket by clicking on the 'Need Help?' link in the upper right-hand corner of the page.
How to Use:
Data Accuracy Counts (top of page)
The ‘Data Accuracy’ section of the page contains counts concerning the zip files and QRDA I documents that have been processed.
Data Accuracy Definitions:
Uploads – The count of all zip files that have been uploaded.
Total Documents – The count of all QRDA I documents contained within the uploaded zip files.
Active Documents – The count of all unique QRDA I documents.  Documents that have been uploaded more than once, whether within the same zip or a different zip, are only counted once as an Active Document.Â
For example, a patient’s QRDA I document for the first quarter is uploaded three times.  It is counted as three Total Documents, but counted as only one Active Document.
Accepted Active Documents – The count of all unique QRDA I documents that have passed the Schematron and Edit rules without generating an error. These documents have been evaluated by the eCQMs and can be submitted to The Joint Commission.
Note:  It is encouraged that Warning messages are reviewed and addressed when possible, as they may point towards potential incorrect eCQM evaluations.  However, active QRDA I documents with only Warning messages may still be submitted to The Joint Commission.
Rejected Active Documents – The count of all unique QRDA I documents that generated at least one Schematron and/or Edit rule error.
Bar Chart ‘QRDA Documents Errors/Warnings’
This bar chart shows a list of all the errors and/or warnings generated by the active QRDA I documents.Â
The bar chart is blank if all active QRDA I documents generated no errors or warnings.
Hovering over a bar, displays the entire name of the error or warning and the number of active QRDA I documents which generated the issue.

Select Type (left side of page):
The ‘Select Type’ allows the user to select the type of messages (error versus warning) they wish to future research.
Uploads (left side of page, under Select Type):
The ‘Uploads’ element lists all zip files containing active QRDA I documents.
Selecting a zip file that has been:
Completely replaced (e.g., all documents contained within the zip have been re-uploaded in more recent zip files) causes a blank bar chart to be displayed and no active QRDA I documents will be available to be selected.
Partially replaced (e.g., some documents contained within the zip have been re-uploaded in more recent zip files) causes a bar chart with appropriate errors and/or warnings to be displayed and the active QRDA I documents that have not been replaced will be available to be selected.
Note:Â In both instances, review the most recent zip file containing the last version of a given active QRDA I document for possible errors and/or warnings.
Active Documents (right side of page):
The ‘Active Documents’ element lists all active QRDA I documents which generated at least one error and/or warning.
Active QRDA I documents lists all active QRDA I documents uploaded.  To review these documents, proceed to the Quality Improvement  page.
QRDA I documents which have been replaced by being re-uploaded are not accessible.  Only the last version of a given QRDA I document is ‘active’ and may be reviewed.  For example:
The QRDA I document for Jane Smith (named ‘Jane Smith QRDA’) is uploaded on 4/1 in a zip file named ‘JS’.
The QRDA I document for Jane Smith (named ‘Jane Smith QRDA’) is re-uploaded on 4/2 in a zip file named ‘JS-2’.
Only the 4/2 version of the Jane Smith QRDA I document is ‘active’ and available to be reviewed.
Clicking on the ‘Magnifying Glass’ next to an active QRDA I document launches the Source Viewer which provides:     Â
the list of Schematron and Edit errors/warnings that the QRDA I document generated along with the ability to view the specific XML file structure of where the error(s) occurred, and
a review of the patient data and how the active QRDA I document was processed against the eCQM logic.
Clear Selection (above the bar chart):Â
The ‘Clear Selection’ button returns the page back to its original display.
Export Errors/Warnings button (right-hand side of page, Next to 'Active Documents'):Â
Clicking the ‘Export Errors/Warnings’ button will take you to a new page that will display how each patient's episode of care processed against the edits and Schematron.
How each element of the page affects the other page elements:
Note:  The counts at the top of the page adjust as the elements on the page are modified.  Clearing these selections returns the counts back to their original state.
