2. To be considered for an exemption from submitting chart-abstracted data for CY2025 requirements, all forms must be received at The Joint Commission no later than thirty (30) days prior to quarter close applicable to the request.

3. To be considered for an exemption from submitting eCQM data for CY2025 requirements, all forms must be received at The Joint Commission no later than the 1Q chart-abstracted deadline. If no eCQMs will be submitted, your organization is required to submit a minimum of three (3) chart-abstracted measures for the entire calendar year, unless the request is to for an exception for submission of any data.

4. Feedback on submitted data is posted to Organization’s Accelerate PI Dashboard in their Joint Commission Connect site approximately three months after the submission deadline.

CY2025 Chart-Abstracted Measures (CAM)

• 1Q 2025 (Jan, Feb, Mar): DDSP opens February 1, 2025 and data must be entered by June 30, 2025

• 2Q 2025 (Apr, May, Jun): DDSP opens May 1, 2025 and data must be entered by September 30, 2025

• 3Q 2025 (Jul, Aug, Sep): DDSP opens August 1, 2025 and data must be entered by December 31, 2025

• 4Q 2025 (Oct, Nov, Dec): DDSP opens November 1, 2025 and data must be entered by March 31, 2026

CY2025 Electronic Clinical Quality Measures (eCQMs)

• CY2025: DDSP opens TBD; Transmission period to submit data opens January 1, 2026, and data must be submitted by March 16, 2026

Specific to eCQMs: To be considered for an exemption from submitting eCQM data for the CY2026 requirements, all forms must be received at The Joint Commission no later than 30 days prior to the eCQM deadline (The eCQM deadline is March 15th, so ECRs must be received no later than February 15th). In cases of unanticipated events (i.e. natural disasters / data breach), organizations should notify us as soon as they are able.

Specific to chart-abstracted measures: To be considered for an exemption from submitting chart-abstracted data for the CY2026 requirements, requests must be received at Joint Commission no later than thirty (30) days prior to quarter close applicable to the request.
