38 | When performing the 'Purge’ and ‘Upload' actions in quick succession can lead to the generation of duplicate rows in the 'Result by Measure' table on the Quality Improvement page, as well as in the Submission Dashboard on the Submit Data page. These duplicates may display the same or varying results. Impact: If data submission is carried out while these duplicate rows exist, only the first row displayed in the table will be stored in the data warehouse. 51 | Numerator Exclusion counts are not displayed correctly in the Waterfall Chart on the Quality Improvement page. | 1/14/2026 | Numerator Exclusion counts are not displayed correctly on the Waterfall Chart within the Quality Improvement page. These cases are incorrectly included within the Numerator bar category. However, both the Numerator and Denominator values are being calculated accurately. The Denominator, Numerator, and Numerator Exclusion counts can be verified using the Review Patient Results option, which can also be downloaded using the Export to Excel feature.
This issue affects only the visual display of the bar chart. It does not impact the underlying counts and has no effect on the hospital’s observed performance measure rate.
